Protocol
AFC has developed the following protocol, designed to mitigate the risk of the spread of COVID-19 while supporting those families and kids who are willing to play.

AFC Mask Considerations - April 2021
All participants must undergo pre-screening before each AFC event (e. g., workout, clinic, practice, or game). The results of the pre-screening are recorded on this form and are kept on file.

Completion of the following waiver is required as a condition of participating in any AFC activities.
